One HDMI cable splitting into 2 tails, with no electronics inside. The signal is copied passively, so quality drops with each extra output.
No electronics: unlike an active splitter, it does not regenerate the signal. A passive splitter also duplicates the DDC/EDID line to both outputs -- if the two connected displays report different real capabilities (resolution/refresh rate), the source can only pick one, which is the real cause of a common passive-splitter complaint ("one screen shows nothing or the wrong resolution").

How it works
The picture goes out on three differential pairs, each carrying the signal and an inverted copy. The display subtracts one from the other, so any noise that hit both wires cancels out. The fourth pair carries the clock that says when each pixel starts.
