How it works
Two independent conductors (left and right) share one ground braid. Neither channel gets noise cancellation, which is why hum and hiss appear after a few metres.

From the published standard
In an unbalanced connection one conductor carries the signal while the braid serves as both ground and shield. There is no inverted copy to cancel noise, so hum and hiss start to appear after a few metres, particularly near mains cable or power supplies. For long runs the answer is a balanced connection, not a longer unbalanced cable.
