A fibre-optic HDMI cable still presents standard HDMI 19-pin connectors at both ends, but the middle of the run converts the signal to light and back -- real benefit over 20m of plain copper HDMI is avoiding the signal degradation copper suffers at that length, at the cost of a fixed signal direction (source end and display end usually cannot be swapped).

How it works
The data (TMDS) is converted to light pulses travelling down a single glass core, so it does not degrade with cable length or pick up electromagnetic interference the way copper does. Separate thin copper wires still carry the 5V power and the slow DDC/CEC control signals, because converting those to light is not practical at their low data rate.

An active optical cable converts the HDMI signal into light inside the connector itself, sends it down glass fibre, and converts it back to electrical at the far end. Ordinary copper loses a 4K signal after roughly 15 metres; light does not have that limit, which is why 40 and 50 metre lengths exist. It is directional, though: the end marked Source must go into the source, and reversing it does not work.
