Standard USB has a real, spec-defined maximum cable length of about 5 metres unshielded -- reaching 45m requires active extension electronics at both ends encoding the USB signal to survive the Cat6 run, not a passive long USB cable.

An extender is a pair of devices: a transmitter at the source and a receiver at the display. The transmitter re-encodes the HDMI signal so it can travel over long, inexpensive twisted-pair network cable, and the receiver rebuilds HDMI at the far end. That sidesteps the short copper limit of direct HDMI. The quoted distance assumes solid-core cable of at least Cat6; thin patch cable or copper-clad aluminium will fall short of it in practice.
